악교정수술에 의한 폐쇄성 수면 무호흡증 환자의 치험례

Abstract
서론
수면 무호흡 증후군 (sleep apnea syndrome)이란 수면 중 상부기도의 폐쇄로 인해 발생하
는 호흡중단이 특징으로 저산소증과 수면분절로 인해 코골이, 주간졸음, 기억력 감퇴, 이른
아침의 두통, 악몽, 우울증 등의 다양한 증상을 나타내며 ,수면중 10초 이상의 무호흡 상태
가 1시간에 5회 또는 7시간 중 30회 이상 발생할 때로 정의한다. 전체인구의 약 1-3% 정도
에서 발생하고 중년남성이나 비만자에게서 호발하며 남성이 여성보다 10배 정도 호발한다고
알려져 있다.
무호흡증(apnea)은 원인에 따라 가장 일반적인 형태인 수면중의 상기도 폐쇄로 인한 폐쇄
성무호흡증, 감소된 호흡근의 활동성으로 인한 중추성무호흡증, 그리고 혼합형무호흡증 등으
로 나눌 수 있다.
진단방법으로는 임상적 검사, 두부방사선 규격사진, 컴퓨터 단층촬영, 자기공명사진 촬영,
수면다원검사(polysomnography)등이 있으며 임상적으로는 먼저 경제발달과 소득증가로 인
한 운동부족과 영양상태의 호전에 따라 계속 증가추세에 있는 비만도 측정을 비롯하여 수면
중의 호흡중단을 야기하는 모든 상기도부위의 비정상적인 해부학적 구조들을 평가해야 한
다.
객관적으로 가장 중요한 진단방법인 수면다원검사는 초기의 진단뿐 아니라 치료의 성공정
도를 평가하는 기준으로도 사용된다. 본 검사에서는 호흡부전지수(RDI), 동맥혈 산소포화도
(SaO2), 뇌파검사(EEG), 심전도검사(ECG lead V2) 등을 해석하게 되는데 호
흡부전지수가 20 이상이거나 동맥혈 산소포화도가 85% 이하로 떨어지면 임상적으로 폐쇄성
수면 무호흡증으로 진단하게 된다.
치료방법은 비외과적 방법과 외과적인 수술법으로 나누어볼 수 있다. 비외과적 방법으로는
체중조절, 지속적 기도양압, acetazolamide, protriptyline 등의 약물치료, 설견인구, head
position holding 장치 등이 이용되고 있으며, 수술적 방법으로는 초창기에 사용된 기관지절
개술(tracheostomy)를 비롯하여 편도척제술(tonsillectomy), 아데노이드 절제술
(adenoidectomy), 코수술(nasal surgery), 구개수구개인두성형술(uvulopalatopharyngoplasty)
이 있으며 최근에는 하악의 전방이동으로 혀기저부를 전방으로 이동시키는 악교정술 등이
널리 사용되고 있다.
이에 저자는 코골이를 동반한 심한 호흡장애를 주소로 본원 이비인후과에서 의뢰된 환자에
게 수면다원검사를 시행한 결과 심한 수면 무호흡증으로 나타나 일차적으로 hyoid
myotomy 및 suspension 없이 악교정수술만으로 양호한 결과를 얻었기에 보고하는 바이다.

Obstructive sleep apnea syndrome (OSAS) is characterized by repeated collapse of the
upper airway, producing hypopnea, apnea, and ultimately, oxygen desaturation of
hemoglobin.
The major characteristics of OSAS are male predominace, obesity, hypersomnolence,
and excessive snoring. Patients suffer deterioration of memory and judgement,
irritability, morning headache, sexual dysfunction, and personality changes. Psychologic
changes involving the cardiopulmonary system occur and may lead to life-threatening
events.
The presurgical evaluation includes nocturnal polysomnography, clinical examination,
radiologic evaluation, and fiberoptic endoscopy.
The current surgical procedures used for OSAS are tracheostomy, tonsillectomy,
adenoidectomy, nasal surgery (septoplasty, partial turbinectomy), tongue reduction,
uvulopalatopharyngoplasty (UPPP), anterior sagittal osteotomy of the mandible with
hyoid myotomy and suspension, and maxillary, mandibular and hyoid advancement.
Selection of the surgical procedure is based upon the severity of the sleep apnea,
presence of a skeletal deficiency, presence of morbid obesity, and anatomic site of the
obstructive process.
Patient with OSAS documented by polysomnography, was evaluated by cephalometric
analysis, and polysomnography before and 6 months following the surgical procedure.
Patient underwent maxillary, mandibular osteotomies via Lefort I, SSRO, and
genioplasty.
